Purpose and Extent
Westlock Alliance Church is collecting and retaining this personal information for the purpose of enrolling your in our program. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Westlock Alliance Church to limit the information collected, or to view your youth's information, please contact us.
Information received is confidential and is being gathered for the purposes of serving your youth while in the care of Westlock Alliance Church. Any medical information collected here serves to authorize Westlock Alliance Church and its staff and volunteers, to obtain medical assistance in emergencies.
Medical Situations
For visiting youth, by default when medically necessary Westlock Alliance Church Youth Ministry Staff will sign consent for medical treatment and authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.
By default you, as legal parent/guardian are required to indemnify and hold blameless the Ministry Staff, Westlock Alliance Church, its Pastors and Board of Elders from and against any loss, damage, or injury suffered by the participant as a result of being part of the activites of Westlock Alliance Church, as well as any medical treatment authorized by the supervising individuals representing the church.
Walking Events
As a visitor to an event with a walking around Westlock component, your youth will be allowed to participate.
Each youth is expected to follow all applicable laws regarding crosswalks and is expected to follow the directions provided by the adult volunteers. By participating in any activity involving walking around town, your youth may risk personal injury or permanent loss.
Driving Events
As a visitor to an event with a driving component, by default your youth will be transported in a motor vehicle driven by an approved driver for Westlock Alliance Church Ministry Events.
Each youth is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follow the directions provided by the driver and aother adult volunteers.
All Events
At all times your youth will be with an approved Westlock Alliance Church Youth Ministry Staff. Safety during events is our primary concern. All necessary precautions will be taken for your youth's wellbeing and protection during all events.
GRANTING CONSENT
By entering your full name below you are agreeing to all statements above and granting official, signed, permission for your youth to participate in the aforementioned events.
