Purpose and Extent: Westlock Alliance Church is collecting and retaining this personal information for the purpose of enrolling your youth in our programs, to assign him/her to the appropriate classes, to develop and nurture ongoing relationships with you and your youth, and to inform you of program updates and upcoming opportunities at our Church. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Westlock Alliance Church to limit the information collected, or to view your youth's information, please contact us.
Information received is confidential and is being gathered for the purposes of serving your youth while in the care of Westlock Alliance Church. Any medical information collected here serves to authorize Westlock Alliance Church, and its staff and volunteers, to obtain medical assistance in emergencies.
Consents (effective September 2026-September 2027)
Medical Consent:
I/we, the parents or guardians (named below), authorize one of the Westlock Alliance Church Youth Ministry Staff to sign consent for my youth (named above) for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above
I/we, named below, undertake, and agree to indemnify and hold blameless Pastor, the Ministry Staff, Westlock Alliance Church, its Pastors and Board of Elders from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of the Westlock Alliance Church, as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in events of the Westlock Alliance Church.
Events outside Westlock Alliance Church Building
The safety of your youth is our primary concern. Precautions will be taken for their wellbeing and protection during all events.
Weekly Bible Study @ Pastor Adam's House or Westlock Alliance (location TBD)
Walking Events: At all times youth will be with an approved Westlock Alliance Church Youth Ministry Staff. I give permission for my youth to participate in walking events. I understand that my youth is expected to follow all applicable laws regarding cross walks and is expected to follow the directions provided by the adult volunteers. I recognize that by participating in any activity involving walking around town, my youth may risk personal injury or permanent loss. I agree to assume any expenses that may be incurred in the event of an accident, illness, or other incapacity, regardless of whether | have authorized such expenses.
Driving Events: At all times youth will be with an approved Westlock Alliance Church Youth Ministry Staff. I give permission for my youth to be transported in a motor vehicle driven by an approved driver for Westlock Alliance Church Youth Ministry Events. I understand that my youth is expected to follow all applicable laws regarding riding in a motor vehicle and is expected to follow the directions provided by the driver and/or other adult volunteers. I recognize that by participating in any activity involving motor vehicle transportation, my youth may risk personal injury or permanent loss. I agree to assume any expenses that may be incurred in the event of an accident, illness, or other incapacity, regardless of whether I have authorized such expenses.
Photos / Videos: I grant permission for the reasonable use of pictures containing my youth (named below) to be taken during youth events. At time these photos may appear on promotional material shown during Sunday PowerPoint presentations / website, or on printed materials for upcoming youth events.
